Initial Comments:
An offsite unannounced follow up survey completed on 10/20/2021, found that, Care 4 U Home Care had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiencies were cited as a result of an unannounced onsite home care agency state re-licensure survey completed on 7/19/21 and a follow up survey completed on 9/28/2021.
